A Health Record Analyst II thoroughly analyzes outpatient and inpatient medical record for complete documentation. Documentation is to include required elements set by Salem Hospital R&R and CMS. If documentation is missing or missing required elements, the analyst will assign a deficiency.

Minimum Qualifications Include:

• Associate's Degree.
• Intermediate Microsoft Office products software skills (Word, Excel, Outlook).
• Two years relevant experience in Health Information Management
• RHIT Preferred

• Requires understanding of Joint Commission, CMS and Salem Health guidelines for chart completion.
• Successfully complete Epic desktop training within the first 90 days of starting the job.
• Certification in Health Information Technology or successful completion of one year accredited Health Information Management program required. Currently, enrolled in Health Information Technology program or Health Information Management program.
• Requires understanding of Joint Commission, CMS and Salem Health guidelines for chart completion.

Knowledge, Skills & Abilities Include:

• Must be able to multi-task to meet deadlines. Ability to work under pressure and with demanding customers.
• Demonstrated ability to read, write, understand and communicate clearly.
• Requires organizational skills including ability to prioritize work to meet deadlines.
• Familiar with scanning software at an Intermediate level
• Comfortable working with multiple software applications to complete assigned job functions

Core Job Functions Include:

1. Thoroughly analyzes the medical record for deficient documentation and if necessary, assign a deficiency.
2. Index- Analysts also assign documentation type to scanned documentation to ensure the chart is accurate for COC
3. Serves as backup for ROI/Workflow Coordinator. Works to ensure that customer ROI needs are met.
4. Cross-trained in other areas of HIM to provide coverage as appropriate: Index, scan, prep, and process TOC’s.
5. Works Epic HIM reports to maintain chart completion accuracy and workflow.
6. Answers and responds to phone calls regarding continued patient care release of information. Utilizes Epic ROI module, Hyper-link reports and fax server functionality to provide patient information to requestor for continued patient care.

Working Conditions Include:

Exposure to weather, heat, cold, chemicals, etc, unusual shifts, etc. Constant sitting, use of arms/wrists, keyboard operation.
