Salem Health is currently seeking a talented MSW Care Manager to join our team!

Works with patients, families, physicians and community providers to evaluate resource/referral needs and coordinate discharge planning. Completes a comprehensive Psychosocial Assessment for each assigned patient and administers MMSE’s per doctor’s order; Coordinates evaluation of involuntary patients. Provides liaison/case management services and brief, supportive counseling to patients and their families in consultation with the treatment team. Has specific knowledge in addressing the emotional, cognitive, behavioral and/or substance abuse needs of adult and geriatric patients. Other duties as assigned.
Minimum Qualifications Include:
• Master’s Degree in Social Work from a CSWE accredited school required.
• LCSW or LMSW required by State of Oregon.
• Required to complete Crisis Prevention Intervention training within the first 30 days after hire and annual recertification thereafter.
Working Conditions Include:
Frequent standing, moving about work area, lift & carry 1-10 pounds, push/pull (force) 1-10 pounds, & walk-level surface.
Constant use of arms/wrist, manual dexterity (grasping/squeezing), & keyboard operation.
Includes exposure to weather, heat, cold, chemicals, etc, unusual shifts, etc.

Unscheduled staff are utilized in periods of increased patient care requirements or high census to provide acceptable staffing levels in addition to providing coverage during a regularly scheduled employee’s absence (sick leave, vacation, etc). This position is not benefits eligible.
Per Diem employees are required to work at least 400 hours per year, however, Salem Health cannot assure a set schedule or hours per week.
